Information for Professional Referrals

Delta Specialty Hospital is the preferred provider of inpatient and outpatient behavioral healthcare treatment in Memphis, TN.

Types of Referral Sources

While potential patients can always contact us directly to make a self-referral, we also welcome referrals from a variety of sources in our community, including:

  • Assisted living facilities
  • Mental health professionals in mental health centers and clinics
  • Psychiatrists
  • Nurse practitioners
  • Recovery centers
  • Sober living houses
  • Independent living facilities
  • Alliance services
  • Mid-South Health Systems (Mobile Crisis)
  • Veteran’s Assistance (VA)

Starting the Referral Process

When you refer a patient or client to Delta, we’ll work with you to ensure timely access to our services. To begin the referral process, please contact our intake center by calling (855) 779-3935. On the call, you’ll be asked to describe the patient’s presenting concerns, and to outline their psychosocial and medical histories if available.

Once we’ve gathered the necessary information, we’ll evaluate the patient’s circumstances and make a determination about admission. Should it be decided that Delta is a good fit for the patient, we’ll begin the admissions process immediately.

Once admitted, the patient signs a release of information that will allow our admissions, medical, and social services staff to collaborate with you on a treatment plan. Together, we’ll work towards the goal of discharging the patient back to the referent’s care unless a different placement is more appropriate.

Criteria for Admission

Admissions at Delta are always considered on an individual basis. However, each patient must meet certain criteria in order to be treated at our center. We treat adults age 18 and above who are struggling with a mental health condition or addiction. Some of the most common mental health conditions we treat include:

  • Schizophrenia
  • Schizoaffective disorder
  • Bipolar disorder
  • Major depression
  • Anxiety disorder
  • Posttraumatic stress disorder (PTSD)
  • Dementia
  • Personality disorders

In addition, we also provide substance abuse treatment services, including a medical detox program, to address addictions to the following:

  • Alcohol
  • Narcotics
  • Opiates
  • Marijuana
  • Polysubstance abuse

Please note that at this time we do not have specialized units for individuals struggling with sexual addiction, eating disorders, or trauma.

Planning for Discharge

Throughout a patient’s time at Delta, we’ll be in touch with you to update you on their progress. We’ll email you and meet with you, and you may participate in therapy sessions and discharge planning by phone, in person, or via Telehealth.

When the time comes for a patient to discharge from Delta, we’ll collaborate with the referent and other providers to ensure a seamless transition out of our care. We’ll create an ongoing treatment plan that outlines recommended next steps for continued success in recovery. Working together, we’ll prevent a lapse in services that could compromise the patient’s health.
